Most hormone pellets last about three to six months. Women often need a new pellet every three to four months, and men every four to six months. The exact timing depends on your dose, your body and how quickly you use the hormone.
What affects how long pellets last?
- The hormone and dose. Testosterone pellets for men are larger doses and often last longer than estradiol pellets for women.
- Activity level. Very active people, and people who sweat a lot or exercise hard, may use pellets faster.
- Metabolism and body composition. Everyone absorbs hormones a little differently.
- Stress and illness. These can change how you feel as levels taper.
Signs it may be time for your next pellet
As a pellet runs low, many patients notice their old symptoms returning: hot flashes or night sweats, poor sleep, low energy, low libido or mood changes. Keep track of when symptoms return so we can fine-tune your schedule. Follow-up labs also guide timing.

Can a pellet be removed early?
Pellets dissolve on their own and are not easily removed, which is why careful, lab-guided dosing matters from the start.
