Testosterone Therapy and Fertility
Testosterone therapy can lower sperm production. If you want children now or later, it is important to plan before starting.
Why it happens
Why TRT affects fertility
Taking testosterone signals the brain to stop sending the hormones that tell the testes to make sperm. Sperm counts can drop sharply, sometimes to zero. In many men this reverses after stopping, but recovery can take months and is not guaranteed.
Common signs
- Lower sperm count on TRT
- Smaller testicles on TRT
- Trouble conceiving
- Low testosterone with fertility goals
- Partner planning pregnancy

Treatment options
Planning ahead
Testosterone therapy is generally not recommended for men actively trying to conceive. Fertility-preserving treatments may be an option, and referral to a fertility specialist may be best. Dr. Bernell will talk through the choices with you.
When testing confirms low testosterone, testosterone replacement therapy may help. Options include EvexiPEL testosterone pellets and other forms of therapy, with regular labs to keep treatment safe. Testosterone is best measured in the morning and confirmed with a second test.
We treat trt and fertility for patients from Kingwood, The Woodlands, Porter, Humble and across the Lake Houston area at our New Caney office. Your first visit starts with lab work and a real conversation about your symptoms.
